For your assistance, the content of the form is as follows:
1. Instructions for completing the form
a. Please complete one form for each expert you wish to nominate.
b. Open this form in a PDF reader that can save completed forms (Adobe Reader can do this).
c. Complete the fields in the form using the Latin alphabet and preferably in English.
d. Save the form to a file. You may edit and save the form as many times as you need.
e. When you have completed the form, send the electronic file to: cbs-16@wmo.int with the subject “CBS-16 nomination”.

The CPD_Person_ID is the unique reference to an individual within the Country Profile Database. Please enter the CPD_Person_ID if the expert is already recorded in the Country Profile Database. The CPS_Person_ID is listed in the pop-up window that appears when you click on the name of the expert in the Country Profile Database.
